  • At least 17 years old.
  • Submitting copy of updated and legalized certificate.
  • Submitting recent photo size 3×4 and 4×6 (4 pieces for each).
  • Submitting ID Card, Family Card, Birth Certificate, and Certificate of Physical and Mental Health.
